Fix of shnid 13443

I made the transition between CDs 2 and 3 seamless by putting all of space onto one track and reverse fading the beginning of the second space track (former d3t01) to make the splice points match. Because I had to do a reverse fade the result isn't perfect, but it is probably not audible; at this point I've already forgotten exactly where it is and will likely never remember. Resulting SBEs were fixed using shntool.
